For anyone requesting an official student record from the Department of Education, you must use the form linked below. You can either print it out and fill it in by hand or fill it out digitally.

Parent/Guardians may login to NYCSA Parent Portal for unofficial transcripts.
Official Educational Records
- Complete the New York City Department of Education Student Records Request Form (linked below)
- Bring the completed request to your guidance counselor

HOW TO REQUEST YOUR RECORDS
Please be advised: Requests must be made using the New York City Department of
Education Student Records Request Form (linked below).
Email Instructions
- Request Form: Attach your completed NYC DOE Students Records Request form.
- Photo Identification: Your request must include a copy of a photo ID that includes a signature.
- Email Subject Line: The subject of your email must be your graduation class.
Example: If you graduated in 1995, the email subject will be "Class of 1995"
- Delivery Location: You must indicate in the email body where you want the Student Records Request to be sent.

Please be aware that the fulfillment of requests may take two to four weeks.
Requests are affected by holidays and school closures.
